Answer to Question 1

C
Since dietary factors contribute to constipation, the patient should be encouraged to increase dietary fiber. Fluid intake should be increased to promote the passage of soft, hydrated stool. Lack of exercise and decreased mobility contribute to constipation. The patient should be encouraged to increase activity, if not contraindicated. Constipation is initially managed by assisting the patient to pass hardened stool or by removing any impacted feces.

Answer to Question 2

D
Evaluating the quality of care provided is a responsibility shared among members of the health care team. Others interested in the outcomes of evaluation include the community and third-party payers.
