When bathing your clients, you should do which of the following to decrease friction and excoriation?
 
  1. Do not use soap.
  2. Use a soft washcloth and towel.
  3. Avoid washing overly dry skin.
  4. Do not use moisturizing lotion.

Question 2

Your client is status post-surgery and has been prescribed bed rest for the next 24 hours. What can you do to prevent the formation of emboli and edema?
 
  1. Turn every 2 hours.
  2. Use positioning devices.
  3. Use sequential compression devices.
  4. Remind the client to move as little as possible so as not to dislodge a clot.
