You make a mistaken on a handwritten chart entry. Which is the best way to correct it?
 
  1. Cover it with correction fluid (e.g., Wite-Out) to keep the notes neat.
  2. Leave it as it is. You cannot correct an error in charting.
  3. Draw a single line through the entry, date and initial it, then make the correction.
  4. Write over the error and initial it.
