You have finished your client's charting for the shift that is the end of the 24-hour period and the bottom half of the page remains blank. What is the generally accepted action to take?

  1. Draw an X through the remainder of the page.
  2. Do nothing since you do not have further notes on the client's care.
  3. This is not an issue because charting is not separated by days.
  4. Draw a single line through the remainder of the page.

Though Candida and Aspergillus species are the most common fungal pathogens causing invasive fungal disease in the immunocompromised, infections due to previously uncommon hyaline and dematiaceous filamentous fungi are occurring more often today. Rare fungal infections, once accurately diagnosed, may require surgical debridement, immunotherapy, and newer antifungals used singly or in combination with older antifungals, on a case-by-case basis.

There are major differences in the metabolism of morphine and the illegal drug heroin. Morphine mostly produces its CNS effects through m-receptors, and at k- and d-receptors. Heroin has a slight affinity for opiate receptors. Most of its actions are due to metabolism to active metabolites (6-acetylmorphine, morphine, and morphine-6-glucuronide).
