When performing a wet to moist dressing, the nurse should:
 
  a. apply a wet dressing to the wound for 30 minutes, remove the wet dressing and then apply a dry sterile dressing.
  b. apply wet soaks and cover with a dry dressing.
  c. irrigate the wound with prescribed irrigant and apply a dry sterile dressing.
  d. gently pack the wound with slightly moist gauze and cover with a dry sterile gauze.
