The transition phase begins when the cervix is dilated 8 cm and ends when the cervix is 10 cm. The client becomes restless and has difficulty following directions. She has increasing rectal pressure, feels warm, and has perspiration on her upper lip. Nausea and vomiting are common during the transition phase. Side lying positioning can promote uteroplacental blood flow.
