Answer to Question 1

A
When pouring a solution into a container on a sterile field, hold the bottle with the label facing upward, about 4 to 6 inches over the container, and pour slowly from the side of the field. This prevents contamination of the label. If the solution is spilled, the field is no longer sterile.

Answer to Question 2

D
Bright red urine color persisting for several hours in the catheter drainage may indicate postoperative hemorrhage. Report hemorrhage, hyperthermia, hypotension, and tachycardia to the physician right away.
