Answer to Question 1

B
Auscultation of the duration, pitch, and intensity of breath sounds at different locations provides additional information about respiratory status. A grating noise on inspiration may be caused from inflammation of the pleura.

Answer to Question 2

D
For a client who has a urinary catheter in place, the drainage bag should remain below the level of the bladder at all times to prevent any backflow of urine.
