Answer to Question 1

C
Before beginning, confirm the client has not consumed any hot or cold food or beverages for the previous 15 minutes, as this will alter the accuracy of the reading.

Answer to Question 2

A
Feedback:
Flurazepam is a hypnotic medication with a very long half-life. It tends to cause daytime drowsiness and increase the risk for falls. The nurse should, therefore, observe the client for falls and injury. Nausea and vomiting, excessive sleep, and gastrointestinal upset may be side effects but are not potential safety hazards.
