Answer to Question 1

A
Nursing responsibilities for a client having a paracentesis performed include maintaining positioning during the procedure, reassuring the client, and applying sterile dressing to the puncture site after a paracentesis has been performed.

Answer to Question 2

D
Feedback:
A dressing is applied only when the port is pierced and the implanted catheter is being used. Implanted catheters remain patent with periodic flushing with heparin. A local anesthetic is applied to reduce the skin discomfort of a client. A percutaneous catheter is used when the client has an active infection. When the client requires extended therapy, tunneled catheters are inserted into a central vein with part of the catheter secured in the subcutaneous tissue.
