Answer to Question 1

B
Priority nursing intervention when cultures and antibiotics are ordered is to ensure the culture sample has been collected prior to starting the first dose of antibiotic medication.

Answer to Question 2

B
Feedback:
The nurse should provide clothing with front zippers, Velcro closures, elastic waists, and oversized buttons and buttonholes to facilitate dressing for the client with a fracture or even just for the elderly client. This will help the client to dress and undress independently. A client with a fracture will find gowns that must be pulled over the head or gowns with hooks or small buttons very uncomfortable to put on or remove independently.
