Answer to Question 1

D
Feedback:
Determining whether the client has had a Pap smear provides insight into the client's health maintenance status and also helps the nurse determine the client's familiarity with this invasive procedure. Pelvic examination is not performed in a side-lying position. The client's urinary elimination and surgical history may be relevant, but these are not primary considerations.

Answer to Question 2

C
Feedback:
Each of the client's two daily doses will be 1/4 tablet, resulting in a daily total of 1/2 tablet.
