Identify the problem and etiology to avoid a circular statement. Such statements are vague and give no direction to nursing care. Less than body requirements and loss of weight is circular. Avoid legally inadvisable statements that imply blame, negligence, or malpractice. The diagnosis that states insufficient prescription of chemotherapy implies that the health care provider gave an inadequate prescription.
