If a medication is prescribed for oral administration and the client's condition has changed, making it impossible for the client to swallow, the nurse must consult the health care provider to prescribe administration by another route.

Despite every effort to administer medications safely, medication errors do occur. Any medication error, whether of omission or commission, must be reported to the health care provider as soon as it is noted and an incident report must be completed.
