Answer to Question 1

D
Guidelines used by nurses for safe administration of medications include administering only medications that have been personally prepared (i.e., not medications prepared by another nurse); listening to questions a client may raise about a medication (e.g., This pill is a different color,) and rechecking the order and the dosage prepared; indicating on the client's MAR and medical record the reason for not administering any medication; seeing that the client actually takes the medication; not leaving any medications at the client's bedside unless specifically permitted by order of a health care provider; and instructing the client to take only those medications prescribed for him or her and not to offer the medications to others.

Answer to Question 2

A
The nurse determines the right time for medication administration by checking the medication administration record (MAR) schedule. Many health care facilities have policies governing timely administration of medications (e.g., that no medication be given more than 1 hour before or 1 hour after its scheduled administration time). Timeliness in medication administration contributes to the effectiveness of the prescribed drug.
