The interdisciplinary health care team usually includes physical and occupational therapists, a speech/language pathologist, recreational therapists, rehabilitation nurses, a physician, social workers, dietitians, and mental health professionals. The members of the team are selected based on the needs of the client. After each discipline assesses the client, a care planning conference is held with the client and the family. This process prevents duplication of services and fragmented care.
