The nurse is caring for a young adult who came to the rehabilitation facility following severe full-thickness burns over 60 of the body to regain function in order to perform ADLs.
 
  After several weeks of therapy, the client states he wishes to leave the facility against medical advice without completing rehabilitation. The nurse: 1. Informs the client that he cannot leave until rehabilitation is completed.
  2. Calls the physician to request a sedative for the client.
  3. Calls the client's family to persuade him to stay in the facility.
  4. Notifies the RN and/or physician that the client would like to leave.

Question 2

When preparing to insert an IV, the nurse reviews the client history to determine if there are sites that have been affected by preexisting conditions.

Rationale: Just as in the acute care facility, the client in the rehabilitation facility has the right to self-determine treatment, and would not be kept from leaving. It would be important to notify the RN and/or physician depending on facility policy, and the client will be asked to sign an elopement form before leaving.
