Answer to Question 1

4,5
Rationale: The nurse handles the barrel and plunger, and does not touch the shaft of the plunger when handling a syringe aseptically

Answer to Question 2

4
Rationale: Muscle spasm pain is caused by the release of chemicals secondary to mechanical stimulation. Jogging does not cause swelling, trauma, or excessive heat. However, the client could have been jogging in extreme heat, or could have twisted the muscle to cause the release of the chemicals into the muscle. The chemical stimulation causes spasm in the muscle, which is very painful.
