Answer to Question 1

3,4
Rationale: Nuchal rigidity, or pain when flexing the neck, is a classic symptom of meningitis resulting from inflammation of the meninges. Positive Kernig's sign is found when the client cannot extend the leg when the hip is flexed at a 90-degree angle. Brudzinski sign, neck flexion causing the knees and hips to flex, would also be positive. Seizures, paralysis, fever, and chills would all be late signs of meningitis.

Answer to Question 2

1
Rationale: The client might need to make diet changes, and the nurse provides client teaching to reinforce the need for the diet.
