Answer to Question 1

1,2,3,5
Rationale 1: A description of the attributes, characteristics and actions of the nurse providing care is essential to a nursing theory

Answer to Question 2

3
Rationale: The nurse would ask the unit clerk to call the chaplain, social worker, or charge nurse to sit with the family and provide support while the nurses are busy providing client care. The unit clerk is not properly trained to support the family, assess coping strategies, or help them understand what is happening. The family should not be told not to disturb the nurses, as they have every right to be concerned.
