Answer to Question 1

A

Feedback
A Pallor is more easily seen in a dark-skinned patient when assessing the conjunctiva, sclera, buccal mucosa, tongue, lips, nail beds, palms, and soles of feet.
B Skin creases and folds are darker than the rest of the body in the dark-skinned patient.
C The hard palate is not a good site to assess for colour changes such as pallor.
D The best site to inspect for jaundice, not pallor, is the sclera.

Answer to Question 2

A
Venipuncture should be performed distal to proximal, which increases the availability of other sites for future IV therapy. The nurse should avoid veins in an extremity with compromised cir-culation, such as a dialysis graft.
The nurse should avoid hardened, cord-like veins.
The nurse should use the nondominant arm, if possible.
The nurse should avoid vigorous rubbing and tapping, which causes injury to the veins.
