Answer to Question 1

A
A concept map is a visual representation of patient problems and interventions that shows their relationship to one another. The primary purpose of a concept map is to synthesize relevant data about a patient, including assessment data, nursing diagnoses, health needs, nursing interventions, and evaluation measures.

Answer to Question 2

D
If you grasp the inside of the gloved hand with the other gloved hand, you have contaminated the original gloved hand. Tucking the glove under the sterile gown contaminates the glove because under the sterile gown is not considered sterile. Grasping the outside of the cuff (sterile) with a nongloved hand contaminates the sterile glove.
