The home health aide implements the plan of care designed by the nurse, and the nurse researches the cost-effectiveness of the plan.

To provide continuity of care, nurses must consider education and referrals in the care of any person admitted to any type of health care setting, and must also involve the client and family in a mutual planning process. Although it is important to be a mentor, role model, and researcher, these roles are not directly related to providing continuity of care.
