Answer to Question 1

D
The first step the nurse should take is to inform the physician that the behavior is both unwelcome and inappropriate, and the physician should be told to stop immediately. It may become necessary to inform the nursing supervisor of the physician's behavior but it is not the best first step for the nurse to take. Calling the medical board would be inappropriate and should not be the first step. Reporting the behavior to the chief of staff would ignore the chain of command and would be unprofessional.

Answer to Question 2

B
Absence of hair on the lower extremities is an indicator of poor perfusion because hair growth requires adequate gas exchange. A blood pressure of 102/64 is lower than the average range, but may be normal for this patient. It would not indicate poor perfusion to the tissues. The patient with heart failure is likely to have a slightly elevated heart rate if not taking medications that slow heart rate because the increased rate compensates for reduced cardiac output. Although shortness of breath may occur with heart failure, this is the result of poor pumping action of the heart, allowing fluid to accumulate in the lungs; it is not an indicator of peripheral circulation.

Dogs have been used in studies to detect various cancers in human subjects. They have been trained to sniff breath samples from humans that were collected by having them breathe into special tubes. These people included 55 lung cancer patients, 31 breast cancer patients, and 83 cancer-free patients. The dogs detected 54 of the 55 lung cancer patients as having cancer, detected 28 of the 31 breast cancer patients, and gave only three false-positive results (detecting cancer in people who didn't have it).
