An 18-year-old woman comes to the obstetrician's office and is informed that she is 6 weeks pregnant. Which statement by the woman is the nurse most concerned about?

One of the most important nursing interventions for promoting maternal and fetal health is to facilitate and teach the importance of early and continuing prenatal care. Early prenatal care can identify complications of pregnancy, prevent some of the effects of maternal disease such as diabetes, and provide an opportunity for patient education. Reading material can be an adjunct to prenatal care; however, this will not allow for maternal and fetal assessments. Prenatal visits can also be the time for the mother to discuss work arrangements as well as babysitting arrangements.

If you suspect child abuse, you are legally responsible for reporting your observations. However as a beginning nursing student, it is best to discuss your concerns and findings with your instructor or the nurse assigned to the client before you make a report.
