Answer to Question 1

ANS: 1
A school-age child will be tired the following day if allowed to stay up later than usual. The nurse should ask a question to assess the child's usual sleep patterns. The nurse should first assess the child's usual sleep pattern to determine if the child is adhering to a bedtime. A sleep problem is often the cause of fatigue.

Answer to Question 2

ANS: 4
As people age, there is a progressive decrease in stages 3 and 4 NREM sleep; some older adults have almost no stage 4, or deep, sleep. Older people do not become more difficult to arouse, not do they require more sleep than the middle-age adult. An older adult awakens more often during the night, and it may take more time for an older adult to fall asleep.
