A patient is taking gentamicin (Garamycin) on once daily regimen. The prescriber orders a blood sample to be drawn for measurement of a trough level of the drug. When should the nurse draw the blood?
 
  a. 1 hour after the infusion is complete
  b. 8 hours after the infusion is complete
  c. 12 hours after the infusion is complete
  d. 1 hour prior to the next infusion

The nurse has just received a patient from the recovery room who has been receiving amikacin (Amikin) for treatment of an infection.
 
  Upon assessment of the patient, the nurse finds that he cannot move his extremities, and he is having difficulty breathing. The nurse suspects neuromuscular blockage and should be prepared to administer

ANS: D
Samples for trough levels should be drawn just prior to the next dose (when divided daily doses are used) or 1 hour before the next dose (when a single daily dose is used).

Answer to Question 2

ANS: B
Neuromuscular blockade can be reversed with calcium. IV infusion of calcium salt (calcium gluconate) is the treatment of choice.
Epinephrine is not indicated for aminoglycoside toxicity; in fact, aminoglycoside toxicity is not apparent.
Decadron would not reverse the current condition.
Nothing indicates that the patient requires magnesium sulfate.
