Answer to Question 1

B
Teaching is most effective when the patient is not distracted or uncomfortable. Lunchtime is a distraction, and so is the mental state of a person who has just been told that he or she has an in-curable health problem (even if it can be controlled). Having four people present with the patient during the first teaching session can be more distracting than helpful.

Answer to Question 2

C
Beta blockers and ACE inhibitors are often used together to treat heart failure. Beta blockers block the effects of epinephrine on the heart resulting in deceased heart rate and force of contrac-tion, thus decreased blood pressure. The nurse should notify the prescriber if the heart rate is less than 60 per minute.

Chronic necrotizing aspergillosis has a slowly progressive process that, unlike invasive aspergillosis, does not spread to other organ systems or the blood vessels. It most often affects middle-aged and elderly individuals, spreading to surrounding tissue in the lungs. The disease often does not respond to conventionally successful treatments, and requires individualized therapies in order to keep it from becoming life-threatening.
