The patient should be instructed to contact the provider if the next dose is not effective. Ergot alkalo-ids should not be given within 24 hours of a triptan. Increasing the next dose to 15 mg would exceed the 24-hour dose maximum.

The result is that the body requires increased drug, or an increased MEC, to achieve the same effect. This patient is getting adequate pain relief, so there is no negative placebo effect. Tachyphylaxis is a form of tolerance that can be defined as a reduction in drug responsiveness brought on by repeated dosing over a short time; this occurs over several months. Barbiturates induce synthesis of hepatic enzymes that cause increased metabolism of the drug, but it does not increase the MEC.
