If a person works all night and goes to bed at 8 AM, arising at 4 PM to carry on the day's activities before going to work at 10 PM, the hypothalamus will release corticotropin-releasing hormone at about 4 PM in accordance with the new sleepwake cycle. It usually takes 2 or 3 days for the hypothalamus to readjust. A patient on this schedule who is taking replacement corticosteroids would then need to take them at 4 PM, or on arising. All other options would not be optimal.
