Duloxetine (Cymbalta) is highly protein bound, extensively metabolized by CYP2D6 and CYP1A2 enzymes in the liver, and excreted by the kidneys. It is not recommended for use in patients with severe renal or any degree of liver impairment. Low hemoglobin, elevated potassium, and increased ESR do not necessarily preclude the use of Cymbalta.
