While assessing the client's IV infusion, the nurse notes that it is infusing slower than it should be. The nurse should first:
 
  A. Discontinue the IV
  B. Increase the rate of the infusion
  C. Observe for fluid overload
  D. Check the position of the IV fluid and extremity

D. Check client for positional change that might affect rate, height of IV container, tubing obstruction. Check condition of site.
A. Discontinue the IV if it is determined that it is infiltrated or clotted off.
B. Position will affect flow even if rate is increased.
C. Fluid overload is not associated with slowing of the infusing rate.
