D. Hypoxia may be caused by poor tubing connections or use of long oxygen tubing. It can also be caused by worsening of client's physical problem with a change in respiratory status. Determine if oxygen delivery device and oxygen source are delivering oxygen properly.
A. Instruct client and family when to notify physician of signs of hypoxia.
B. Semi-Fowler's position will relieve any pressure on the diaphragm.
C. Instruct client not to change oxygen flow rate.

D, B, and C. If the dorsalis pedis pulse is difficult to palpate or it is not palpable, use a Doppler instrument over the pulse site: Doppler amplifies sounds, allowing nurse to hear low-velocity blood flow through peripheral arteries.
A. Report to nurse in charge or physician acute pain or sudden muscle weakness, which may be indicative of condition requiring immediate treatment.
