A. Adherent film is used on a pressure ulcer with eschar; it facilitates softening of the eschar.
B. Hydrogel foam is used in clean stage III and IV pressure ulcers to protect the wound and absorb moisture; it would not be used with eschar.
C. Composite film would be used on a clean stage II pressure ulcer to limit shear and facilitate reepithelialization .
D. Calcium alginate is used on stage III and IV ulcers that have significant exudates; it is highly absorbent. It would not be used on an ulcer with eschar.
