Answer to Question 1

C
C. Using a clean washcloth, wipe in circular motion along length of catheter for about 10 cm (4 inches).
A. Allow slack in catheter so movement does not create tension on catheter.
B. Empty drainage bag and record amounts at least every 8 hours.
D. Drainage bag must be below level of bladder; do not place bag on side rails of bed.

Answer to Question 2

D
D. Before assessment a specially trained technician will conduct a radiation survey of the client, initially conducting a scan of the face, hands, and feet using a radiation survey instrument. If meter results are positive, a thorough survey (5 to 8 minutes per person) is conducted).
A. Wash client's skin thoroughly with water and soap, taking care not to abrade or irritate the skin. Do not allow radioactive material to be incorporated into any wounds.
B. Remove client's clothing; 70 to 90 of contamination should be eliminated by removal of clothing. Use of copious amounts of water is critical in decontamination.
C. Isolate and cover any area of the skin that is still positive for radiation, by using a plastic bag or wrap.
