Answer to Question 1

A
Temperature is lowest during early morning. Muscle activity and stress raise heat production. Drugs may impair or promote sweating, vasoconstriction, or vasodilation, or may interfere with the ability of the hypothalamus to regulate temperature.

Answer to Question 2

D
D. Draw a single line through the error, write the word error above it, and sign your name or initials. Then record note correctly. Check agency policy.
A. Do not scratch out errors. Charting becomes illegible.
B. The nurse should never erase entries or use correction fluid and should never use a pencil. The use of correction fluid could make the charting become illegible, and it may appear as if the nurse were attempting to hide something or to deface the record.
C. Because the record is a legal document, you would not destroy part of the record. It is also possible that you are destroying another nurse's record.

There are major differences in the metabolism of morphine and the illegal drug heroin. Morphine mostly produces its CNS effects through m-receptors, and at k- and d-receptors. Heroin has a slight affinity for opiate receptors. Most of its actions are due to metabolism to active metabolites (6-acetylmorphine, morphine, and morphine-6-glucuronide).

Your skin wrinkles if you stay in the bathtub a long time because the outermost layer of skin (which consists of dead keratin) swells when it absorbs water. It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
