When caring for clients with skin disorders, the nurse should:
 
  a. avoid assessing skin lesions so as not to embarrass the client.
  b. encourage the client to hide the skin condition under clothing.
  c. tell the client not to worry about how he or she appears, because it's what's inside that counts.
