What should a good informed consent form include?
 
  a. Clearly state the risks and benefits that are part of participation in a
  study.
  b. Be less no less than a page and no more than 2 pages in length.
  c. Ask for the participant's contact information.
  d. Ask the participant to waive his/her future legal rights.
