Cogeneration is used when there is a reason for producing industrial steam for
manufacturing processes. If fuel is being burnt to boil water, why not also generate
electricity? (This was not allowed by regulators, who gave utilities monopolies.)

It would seem to be the case that we are near the ultimate limit to the number of
people that can live on the planet. See the estimates in the chapter; many are around the
present population. The greatest is some 80 billion, fewer than four doubling times from
now.
