If attorneys have not stipulated that the signature of the person giving testimony is
   waived, you must:
 
  a. submit proof along with the deposition that the signature is waived
  b. assume that the stipulation was forgotten and therefore not get signatures
  c. obtain the signature of the witness
  d. ask the attorney who did the questioning to get the required signature
