Answer to Question 1

Do you solemnly swear that the testimony you are about to offer regarding this cause hereinunder consideration shall be the truth, the whole truth, and nothing but the truth, so help you God?

Answer to Question 2

1 . The taking of the deposition before any notary or before the assigned court reporter; 2 . All objections are waived except the form of the question; 3 . The reading and/or signature of the deposition by the witness is waived; 4 . That the signature of the witness may take place before any notary; 5 . The filing of the deposition is waived; etc.
