If you were to light for a commercial for a new fuel-efficient hybrid car, what type of
  lighting would you choose? Why?

  How would you light this scene to communicate that the room is unsanitary and that
  the patient is not well cared for and is getting worse? How would you light the scene to
  show that the patient is well cared for in a clean environment and that he or she is definitely
