What is the standard for admissibility under Miranda? How do these three questions incorporate the concept of voluntariness?

The standard for admissibility is three questions: (1) Were the Miranda warnings
given? (2) If they were given, was there a waiver? (3) If there was a waiver, was it
intelligent and voluntary? Voluntariness is assumed from a yes answer to all three
questions.
