ANSWER: Name and address of child, childs age, nature and extent of the injuries or abuse, childs present location, any disclosure statement the child may have made, person making the call and where they can be reached

ANSWER: The teacher should listen carefully, jot details if possible, ask no leading or probing questions, write down any questions asked and responses, and report immediately according to the policies of the school or program. The teacher should assure the child that he or she will be protected.
