The patient is a 34-year-old female who gives birth to her fourth child via normal vaginal delivery with no complications. She and her husband have decided they are through with child bearing. She has opted for permanent sterilization and will be having a tubal ligation while in the hospital for the delivery. Prior to pregnancy, she had normal menstrual periods and no pre-existing medical conditions. The risks and benefits of the procedure were discussed with the patient, including the fact that once the procedure is done, she will no longer be able to conceive children; sterilization is a permanent procedure. Stating that she fully understands the consequences and reiterating that she is refusing other forms of birth control, she consents to the procedure.

 Dx: normal delivery, single liveborn infant; permanent sterilization
