Provide the three codes that would be given for the following scenario: "A patient sees the physician for a physical examination. During the exam, the physician determines that the patient is overweight, a condition that can make a patient prone to develop diabetes. Her BMI is 28. The patient also has a family history of diabetes mellitus. Even though the physician determines that the patient does not have diabetes, he counsels her on better eating habits to lose weight. Because the patient also has a family history of diabetes, the physician tells her that she is more likely to become diabetic than someone without a family history. He provides her with additional literature on diabetes with suggestions to better manage her health."
