Mr. Lewis is seen prior to a sleep study for assessment of his sleep apnea and insomnia. He states that his sleep apnea has improved somewhat with the CPAP, and he feels more rested when he awakens. He reports no change in his insomnia. He will present to the sleep lab at Memorial Hospital this evening for his scheduled overnight sleep study.

She is finding it more difficult to perform her work duties as a result of her symptoms. Due to the patient's symptoms and history, she is given a working diagnosis of chronic fatigue syndrome. I offered to help complete any medical paperwork that she may need in order to request time off from her employer.
