Question 1

Vital signs: temperature 98.4, respirations 16, BP 119/81. General: A well-developed, well-nourished female in no apparent distress. HEENT: Normocephalic, atraumatic. Mucous membranes appear moist. Lungs: Clear. Heart: Regular rate and rhythm. Abdomen: Positive bowel sounds, soft, nontender, and nondistended. Back: No swelling, bruising, or erythema. Extremities: Lower extremities show no muscle wasting. Patient has 2+ patellar and ankle jerks, 5/5 muscle strength bilaterally, and a negative straight leg raise bilaterally. ____________________



Question 2

An alert, 16-year-old male. Temperature 98.6, pulse 86, respirations 14, BP 126/80. He has a 2cm laceration of the chin. ____________________
