Proliferation of mutated myeloid stem cells is the cause for myelodysplastic syndromes. Apoptosis is increased in early myleodysplastic syndrome, giving rise to the cytopenia (or cytopenias); however, later, if the disease progresses and acute leukemia appears, then apoptosis decreases. They are not inherited, and no association exists with the Epstein-Barr virus.

Maturation of granulocytes is abnormal in several of myelodysplastic syndromes, and as a result hypogranular or agranular neutrophils may be seen.
