4. The client with a high anxiety level should be provided with a calm, quiet environment in order to reduce stimuli.

1. Humor is an attempt to reduce stress by making the situation amusing. Denial is a defense mechanism that refuses to acknowledge the stress at all.
2. A client regresses to a time in one's life when stress was less intrusive.
3. Sublimating involves redirecting unacceptable behavior into acceptable behavior.
4. The client asks the nurse why defense mechanisms are used, and the nurse would tell the client that the behaviors help the client deal with stress. Some are healthy and some are not healthy.
